The School Based Health Center Collaborative Mission:

The Collaborative aims to improve and support student health and academic outcomes by building an innovative and effective School-Based Health Care (SBHC) model. We strive to become a leader among SBHCs as we revolutionize school health services. Our mission-aligned staff innovates and collaborates, building trust and relationships with communication and expertise. Our mission is student-centered and we are setting them up for a lifetime of success with a focus on health equity.

Beginning in late November 2021, FPCN is partnering with Education Plus Health to offer school-based health services to students in several schools around Philadelphia. Education Plus Health is a student-centered organization with a focus on health equity whose services and activities aim to improve outcomes for student health and academic achievement. Education Plus Health and FPCN will work together to enhance services to Philadelphia students, ensuring that high quality primary care, behavioral health and dental care is delivered to students in a trauma-informed, culturally competent and compassionate manner. The partnership will eventually have a presence in six schools across the city. The Collaborative’s Beliefs:

We believe that health care must be delivered in a culturally competent, trauma-informed and compassionate manner. We are accepting appointments for the following services; however they are not offered onsite at the Mill Creek location. We are happy to coordinate a referral at one of our other network locations.

Health Annex is located in Southwest Philadelphia and joined the FQHC network in 2003 after being part of University of Pennsylvania’s practice prior to that. Health Annex provides a range of services to patients, including Primary Care and integrated Behavioral Health, Outpatient Behavioral Health, Dental Care, Social Work, Outreach & Enrollment, Diabetes and Nutrition education and support, Mind/Body practices, prenatal care, specialist services, physical therapy, on-site pharmacy, transportation assistance and a fitness room. The site also has a community room and kitchen that are often used by local groups interested in partnering with Health Annex to host events or educational activities for local residents. Health Annex underwent a large expansion in 2007 in order to boost its capacity to work with even more community members, providing essential healthcare and other supportive services.

The Health Annex is currently closed for renovations.
