After two years of serving the community in a temporary space as result of the civic uprisings commemorating Mr. George Floyd, Family Practice & Counseling Network (FPCN) is excited to return back home to our Southwest Philly community with the reopening of the Health Annex clinic! We thank you for your patience and for your continued support during this time and we look forward to continue serving the Southwest Philly and surrounding communities.

Health Annex is located in Southwest Philadelphia and joined the FQHC network in 2003 after being part of University of Pennsylvania’s practice prior to that. Health Annex provides a range of services to patients, including Primary Care and integrated Behavioral Health, Outpatient Behavioral Health, Dental Care, Social Work, Outreach & Enrollment, Diabetes and Nutrition education and support, Mind/Body practices, prenatal care, specialist services, physical therapy, on-site pharmacy, transportation assistance and a fitness room.

The site also has a community room and kitchen that are often used by local groups interested in partnering with Health Annex to host events or educational activities for local residents. Health Annex underwent a large expansion in 2007 in order to boost its capacity to work with even more community members, providing essential healthcare and other supportive services.
